Opened in December 2008, Greystone Manor is an assisted-living residence in Lakeville, Minnesota
for up to five compatible seniors in a home-style setting. Nestled in a quiet cul-de-sac on a hill, Greystone
Manor offers easy accessibility from Cedar Avenue and from 160th Street. Greystone Manor
always has one or more care staff on site, twenty-four hours a day, seven days a week to provide assistance with personal
care needs, companionship, games, and activities. Greystone Manor offers a warm and inviting ambiance
for seniors who may need a little extra support, assistance, or supervision; and who want the companionship, security, and
comfort of a home setting.

Greystone
Manor care staff members focus on what each resident can do, and help the residents find easy ways to achieve success
and balance in their daily lives. Our goal is to have our residents be happy, active and involved in day-to-day activities.
Greystone Manor offers affordable care with options for residents to age in place - we can provide
additional and advanced care services on-site without the resident's need to relocate to another facility. Our
tiered pricing allows residents to obtain an appropriate suite of services without paying for a more comprehensive level of
care that may not yet be necessary at their point in life. Additional personal care, health care, nursing services,
and Medicare services are available. Greystone Manor care is provided by Professional Resource Network
Senior Care (PRNSC, LLC) which is licensed by the State of Minnesota Department of Health as a Class A Home Health Care Agency.

Greystone Manor basic monthly cost includes a private bedroom, assistance with activities of daily
living, medication set-up, medication reminders, on-site care staff 24 hours a day, 7 days a week; cable television, utilities,
laundry, housekeeping, activities, and all meals. Greystone Manor visits are available by appointment
